In 2026 the standard Part B premium is $202.90 a month. Above $109,000 of 2024 MAGI for an individual return, or $218,000 for a joint return, Medicare adds a surcharge to Part B and Part D in five steps.

Individual and joint returns

Individual returns cover single filers, heads of household, qualifying surviving spouses, and married people who file separately and lived apart for the whole year. Joint limits are exactly double the individual limits up to the top tier.

Married filing separately

If you are married, file separately and lived with your spouse at any point in the year, there are only three levels, and the first surcharge is the fourth-tier amount. This is why filing separately rarely saves money for Medicare enrollees.

How to use this table

Find your MAGI from your 2024 federal return: adjusted gross income on line 11 of Form 1040 plus tax-exempt interest on line 2a. Match it to your filing status. The yearly column is what one person pays for Part B plus the Part D surcharge; a married couple both on Medicare pays it twice. The Part D surcharge applies even if your drug plan premium is $0, and it is usually deducted from your Social Security check along with Part B.

Each limit is a hard edge. A MAGI of $109,000 pays the standard premium; $109,001 pays tier one for all of 2026. If you are close to an edge for a future year, the Roth headroom finder shows how much income you can still add, and the main calculator lets you compare 2026 with the projected 2027 brackets.

What changed from 2025

The standard Part B premium rose from $185.00 in 2025 to $202.90 in 2026. The income limits rose with inflation, from $106,000 to $109,000 for individual returns and from $212,000 to $218,000 for joint returns. The top limits of $500,000 and $750,000 are fixed by law and do not change.
