2027 IRMAA brackets
Your 2027 Medicare premiums will be set by your 2025 tax return. Medicare publishes the official limits and premiums in the fall; until then, these projected limits help you see whether 2025 income is likely to trigger a surcharge.

Projected 2027 limits
The premium amounts below repeat the official 2026 figures because the 2027 premiums are not known yet. Expect them to rise when Medicare announces them; the income limits are the part you can plan around now.

How we project the limits
The first IRMAA limit is adjusted each year for inflation and rounded to a multiple of $1,000. We project the 2027 starting point at $112,000 for individual returns and $224,000 for joint returns from published inflation data. The middle limits are the 2026 limits scaled by the same ratio and rounded to the nearest $1,000. The top limits of $500,000 and $750,000 are written into the law and do not move with inflation, so they stay fixed. The married-filing-separately limit is the top limit minus the first limit.
This is an estimate, not an official number. The methodology page shows every input and the date we computed it. When Medicare publishes the real figures, we replace this page with them within a week and remove the projected label.
Why 2025 income matters now
Because of the two-year lookback, your 2025 return is already final or nearly final for most people, so the main use of this page is to know what is coming. If 2025 included a one-time spike, such as a home sale or a large Roth conversion, expect a higher premium in 2027 and plan your cash flow for it. If the spike came from a life-changing event, such as retiring or losing a spouse, you may be able to ask for a new decision with Form SSA-44; the appeal checklist walks you through it.
For income you can still shape, such as 2026 and 2027 conversions, the Roth headroom finder shows how much you can add before a cliff.
